  1.  A New OnTrak TesTcup device must be used for each test.
  2.  A minimum of 30 mL of urine sample is required to run the test.
  3.  OnTrak TesTcup is packaged with an expiration date and lot number on 
                the outside of the bag.
  4.  OnTrak TesTcup devices can be used up to 12 hours after removing from 
                the bag.
  5.  Room temperature samples will run quicker than fresh samples.
  6.  OnTrak TesTcup will leak if turned upside down.
  7.  OnTrak TesTcup should be refrigerated prior to use.
  8.  The preferred amount of urine to collect is 120 mL.
  9.  OnTrak TesTcup devices can be used after their expiration date as long 
                as the bag has not been opened.
  10.  One of the two tabs on the lid must be aligned with the blue "test" 
                  arrow prior to tilting.
  11.  Sample should cover 1/2 to 3/4 of the lid when tilting the cup.
  12.  OnTrak TesTcup should be put in the lock position prior to running the test.  13.  OnTrak TesTcup should be tilted for 3 seconds.
  14.  OnTrak TesTcup should be placed upright and the lid turned to the 
                  locked position after tilting.
  15.  The result label should be discarded after removing from the cup.
  16.  Test results can be interpreted once the test valid bands form.
  17.  Any information of a blue band in the test result area indicates a 
                  positive test.
  18.  OnTrak TesTcup can not be run once it has been placed in the 
                  locked position.
  19.  Wait 5 minutes after collecting the sample before running the test.
  20.  To ship or transport an OnTrak TesTcup device place lid in locked 
                  position and place result label on back of cup over the vent hole.
